MANDATORY INTAKE FORM(S)

Your intake form(s) MUST be completed within 24 hours of the session time.  Your appointment CANNOT proceed without this completed form.

For 1 Person Sessions:

Complete your intake form immediately using the link provided. Your appointment cannot proceed without this completed form.

For 2 Person or Group Sessions: 

IMPORTANT: Each participant requires their own individual intake form to be completed.  The client must complete a separate form for every recipient

Example:

4 people attending = 4 separate intake form submissions required. 

Complete Form 1 with Person A’s details → Submit 
Complete Form 2 with Person B’s details → Submit
Complete Form 3 with Person C’s details → Submit
Complete Form 4 with Person D’s details → Submit

IMPORTANT POLICIES

SESSION CONFIRMATION

  • Sessions will ONLY proceed once ALL required intake forms for the session are received 

PAYMENT & CANCELLATION

  • Payment due prior to scheduling
  • Full refund for cancellations 24+ hours in advance
  • Same-day cancellations are non-refundable
  • Sessions without completed intake forms will be cancelled with full refund
  • We reserve the right to decline service with full refund
  • Refunds processed within 5-7 business days. 
  • Intake form reviews are completed within business hours
